In the Calgary Zone, community collections will stop at the following locations:

• Alberta Children’s Hospital
• Foothills Medical Centre
• Okotoks Health & Wellness Centre
• Peter Lougheed Hospital
• Rockyview General Hospital
• South Health Campus
• Strathmore District Health Services

DynaLIFE is also opening new Patient Service Centres (PSCs) in the Calgary Zone at:

• Okotoks PSC (201, 200 Southridge Drive, Okotoks)
• Strathmore PSC (Bay H, 510 Hwy 1 (Country Lane Mall), Strathmore)

Before December 5, patients can continue booking appointments on their current patient appointment booking system.

From December 5 onward, to book their community lab services appointments patients will either:

• Create an account in the qme.dynalife.ca appointment booking system or book as a guest.
• Call the DynaLIFE Appointment Booking Line (1-877-702-4486) if you are having issues or would prefer to book over the phone.

Note: After December 5, all patients in Okotoks and Strathmore with appointments booked at current APL locations will need to attend their appointments at the new DynaLIFE Patient Service Center location in their community (as listed above).
